What is it?
Remote Patient Monitoring uses connected devices—like blood pressure monitors, pulse oximeters, weight scales, and glucose meters—to track vital signs and health metrics in real time. The data collected is then shared with Clinical Monitoring teams, who assess the client's condition and take timely action to improve health outcomes, reduce hospital readmissions, and prevent complications.
How does it work?
Clients receive a tablet and connected devices that sync with the system to track their health metrics. This real-time data allows for video consultations, access to personalized care plans, and ongoing communication with the care team, enabling clients to manage their health from home.

What is it?
Remote Therapeutic Monitoring (RTM) services use digital tools (apps, sensors, inhaler trackers, etc.) to remotely track clients’ therapy adherence and response to therapy. RTM facilitates proactive management of chronic conditions. It has also been shown to facilitate proactive management of chronic conditions, enhances treatment adherence, improves patient outcomes, reduces hospital visits and supports long-term health improvements.
How does it work?
Our network of RTM health providers which include physical therapists (PTs), occupational therapists (OTs), speech-language pathologists and physiatrists utilize digital health tools to monitor, track and report our clients’ therapeutic data.
